ABSTRACT

Objective To investigate the influence of cognitive behavioral intervention on self efficacy and quality of life in patients with chronic hepatitis B (CHB).Methods 90 patients with chronic hepatitis B were randomly divided into cognitive behavioral intervention group and control group,45 cases in each group.The patients of the control group were treated with routine treatment,the intervention group was given cognitive behavior intervention on the basis of conventional treatment.The general self efficacy scale (GSEs) and SF-36 quantity form (SF-36) were used to evaluate the effects of intervention.Results Before the intervention,the GSES score and SF-36 scores of each dimension of the two groups had no statistically significant differences (all P > 0.05).After intervention,the GSES scores of the intervention group was (22.53 ± 4.12) points,which was significantly higher than (17.82 ± 4.51)points of the control group,there was significant difference between the two groups (t =4.918,P <0.01).The dimensions of the intervention group SF-36 score[comprehensive health status (16.13 ± 2.04)points and physical function (17.73 ± 3.49) points,mental health (15.73 ± 2.69) points,role limitations (18.38 ± 2.78) points,social function (14.76 ± 2.96) points,physiological (15.89 ± 2.85) points,vitality and energy (19.18 ± 3.43) points,body pain (19.84 ± 3.78) points] were significantly increased,and compared with the control group [general health status (12.62 ± 2.15) points and physical function (13.18 ± 2.31) points,mental health (9.24 ± 3.54) points,role limitations (8.67 ± 3.47) points,social function (9.24 ± 2.42) points,physiological (8.67 ± 2.60) points,vitality and energy (10.64 ± 2.73) points,body pain (10.80 ± 2.40) points],the differences were statistically significant (t=6.896,8.863,9.189,17.309,9.287,12.046,11.645,14.937,all P<0.05).Conclusion Cognitive behavioral intervention can significantly enhance the self-efficacy of patients with hepatitis B,and improve the quality of life of patients,it can be popularized in clinical work.

ABSTRACT

Objective To study the relevance between the polymorphisms and haplotype of multidrug resistant gene (MDR1) and the ratio of blood concentration/dosage in individuals treated by tacrolimus in stable phases post renal transplanta-tion ,and to provide data for personalized FK-506 administration .Methods The MDR1 C1236T ,G2677T/A and C3435T gen-otypes of 104 renal transplantation patients were determined by PCR followed sequencing method .The blood concentration of tacrolimus was detected by EMIT method .The differences in concentration/(dose × weight) (C/D) ratios were compared a-mong all of the genotype groups treated by tacrolimus .Results In the 104 renal transplantation recipients ,the frequency of MDR1 C1236T ,G2677T/A and C3435T mutation alleles was 56 .73% ,55 .77% and 33 .17% ,respectively .A correlation was revealed among SNP of MDR1 C3435T ,TTT haplotype and the C/D ratios (P<0.05) .In patients of CYP3A5*3*3 ,A cor-relation was also found among TTT haplotype and the C/D ratios (P< 0.05) .No difference was found among the MDR1 C1236T ,G2677T/A ,CGC haplotype and the C/D ratios (P>0.05) .Conclusion It is demonstrated that genetic polymor-phisms of MDR1 C3435T and TTT haplotype are correlated with C/D ratio of tacrolimus in Chinese renal transplant patients in stable stage .These findings affect the dose-adjusted concentration of tacrolimus individually .

ABSTRACT

Objective To analyze the utilization of antibiotics for special use in our hospital in order to provide references for rational use antibiotics in clinic .Methods The utilization of antibiotics for special use in our hospital from 2010 to 2011 was analyzed with defined daily dose (DDD), consumption sum, DDC, DDDs and B/A as indexes.Results 72.22%of antibiotics for special use had been used in our hospital , main in injection .The changes of consumption sum , DDDs and proportion , DDC of antibiotics for spe-cial use varied from each other .The ratio of B to A of antibiotics for special use was in a range of 0.29~9.00.Conclusion The per-formance of stratified management of antibiotics did not implement exactly .It was necessary to strictly control the introduction , intensify education of medication and rigorously implement the performance of stratified management of antibiotics .
